Pre-Admission Packet
(Pink Packet)
              Registration


At Baylor Jack and Jane Hamilton Heart and Vascular Hospital, we strive to make your hospital stay go as smoothly as possible. To assist you in preparing for your stay, we have developed a Pre-Admission Pink Packet. The Pink Packet provides us with your medical history and personal preferences.

Most patients will receive a Pink Packet and information about the Online Pre-Registration process from their referring physician prior to the date of the scheduled procedure. We ask that you bring the completed Pink Packet forms with you when you arrive at Baylor Jack and Jane Hamilton Heart and Vascular Hospital on the day of your procedure. By completing the Pink Packet forms and Online Pre-Registration prior to your visit, the admissions process moves quickly and efficiently. This will also allow our staff time to review your information and provide reduced wait times during the registration process on the day of your admission.

Please submit your Online Pre-Registration form at least two business days before your scheduled procedure. If less than two business days, please call our Access Services department at 214.820.0620 during the business hours of 8:00am until 4:30pm to complete the pre-registration process.

In the Pink Packet, you will find:


  • Admission Database Information. This form includes information about previous surgeries, your medical history, a pain assessment, and questions about health management. The medication section is very important and should include a listing of the medications that you take regularly, along with the dose and frequency of each medication. It is important for you to tell us all of the over-the-counter medications you are taking, including herbal preparations, homeopathics, vitamins, and supplements. If you were asked to stop taking a medication for a few days prior to arriving, please let us know. In addition, please identify all allergies that you may have. It is vital that you complete this section, listing any medications to which you are allergic. You should also include latex and shellfish allergies, if applicable.
  • Advance Directive Tracking. An Advance Directive Form allows you to communicate your decisions about your care if you become unable to make medical decisions. An Advance Directive describes the kind of treatment you would want, depending on how ill you are, and whom to contact should you no longer be able to make a decision. Samples of advance directives include: a living will, a medical power of attorney, and a do not resuscitate order.
  • Pre-Admission Information for Patients. This is a handy checklist to help you prepare for your hospital visit. This checklist includes a listing of the things that you need to do to prepare for your admission, what you need to bring, what to do on the day of the procedure, and any special instructions.
  • Safety First Hotsheet. Your safety is very important to us at Baylor Hamilton Heart and Vascular Hospital. This form will alert us to your diabetic needs, shellfish or latex allergies, and whether or not you have an implanted device, such as a pacemaker or defibrillator. We would also like to know if you are on BIPAP or CPAP, and request that you bring your personal machine.
  • Primary Care Physician Information. This form will give us the necessary contact information for your primary care physician. Our teams at the Baylor Hamilton Heart and Vascular Hospital work closely with your primary care physician, and this will help us be in contact with him or her.


When filling out the Pink Packet, please use black ink and write legibly. Bring the Pink Packet with you on the day of admission.

You should bring all of your prescription medications, herbal preparations, supplements, and vitamins with you to the hospital in their original containers. It is important for us to know how you have been taking them and the instructions that were provided with your prescription medications. Your medications will be safely stored until you are discharged, or they will be sent home with a family member. You will not need these medications in the hospital. All of your medications will come from our hospital pharmacy.
