Enhanced visitor precautions, revised August 18

Baylor Scott & White Health has asked for family and visitors to understand that we have transitioned to a one visitor per patient restriction upon the guidance from the Center for Disease Control and Prevention, the Centers for Medicare and Medicaid Services and federal, state and local authorities to protect the health and safety risk of patients and staff during the COVID-19 pandemic.

Any approved visitor must be 16 years of age or older. Approved exceptions must pass a health screening at entrance. Additional details may be provided by scheduling or registration personnel. Upon arrival, patients and visitors will be screened using CDC guidelines.

At Baylor Scott & White Heart and Vascular Hospital, we strive to make your hospital stay go as smoothly as possible. To assist you in preparing for your stay, we have developed a Pre-Admission Pink Packet. This is called the Pink Packet as we have placed forms and information needed for your stay in a large pink envelope. You should have received your Pink Packet at your physician's office.

Most patients will receive a Pink Packet from their referring physician prior to the date of the scheduled procedure. We ask that you bring the completed Pink Packet forms with you when you arrive at Baylor Scott & White Heart and Vascular Hospital on the day of your procedure. By completing the Pink Packet forms prior to your visit, the admissions process moves quickly and efficiently. This will also allow our staff time to review your information and provide reduced wait times during the registration process on the day of your admission.

What’s in the Pink Packet


Admission database information

This form includes information about previous surgeries, your medical history, a pain assessment, and questions about health management.

The medication section is very important and should include a listing of the medications that you take regularly, along with the dose and frequency of each medication. It is important for you to tell us all of the over-the-counter medications you are taking, including herbal preparations, homeopathics, vitamins, and supplements. If you were asked to stop taking a medication for a few days prior to arriving, please let us know.

In addition, please identify all allergies that you may have. It is vital that you complete this section, listing any medications to which you are allergic. You should also include latex and shellfish allergies, if applicable.

Pre-admission information for patients

This is a handy brochure to help you prepare for your hospital visit. This brochure includes a listing of the things that you need to do to prepare for your admission, what you need to bring, what to do on the day of the procedure, and any special instructions.

Primary care physician information

This form will give us the necessary contact information for your primary care physician. Our teams at the Baylor Scott & White Heart and Vascular Hospital work closely with your primary care physician, and this will help us be in contact with him or her.

Preparation tips

  • Use black ink and write legibly
  • Bring the packet with you on the day of admission
  • Bring your current driver’s license and insurance card
  • Remember to bring any medications that you are taking

  • Contact information is available for the hospital or one of the physicians on our medical staff to answer any questions you may have.

  • Review your health information and test results from the hospital and Health Texas Provider Network physician offices (all historic records may not be available) (not all health information in your medical record is available online. For a complete copy of your medical record, contact the health information management department at the Baylor Scott & White clinic or facility you last visited.)
  • Schedule appointments with your provider or cancel and reschedule upcoming appointments
  • Update your email or mailing address
  • Communicate with your doctor's office using secure messaging
  • Review and pay medical bills
  • Manage your family members' health information (permission required)
